FD-258 fingerprint cards for travel nurses and out-of-state licenses

The FD-258 is the standard FBI applicant fingerprint card. We roll your prints on FD-258 cards for travel nurses and other professionals applying for licenses in other states, ATF paper submissions, immigration and visa applications, and Oregon expungement motions.

Independent private service

Oregon Apostille Fingerprinting Notary is a private fingerprinting service and is not affiliated with the FBI or any licensing board or government agency. Your agency or board processes your background check.

When you need FD-258 cards

Use FD-258 cards when an agency asks for hard-copy fingerprints, such as a paper ATF submission, some visa and immigration programs, and licensing boards in other states.

Other cards that require ink

Besides the FD-258, we roll ink prints on the FBI's FD-1164 card, used to request your own FBI Identity History Summary, and the SF-87 fingerprint chart that some federal agencies require. We also complete state-specific cards for Ohio, Maryland, and New Jersey, and any other card your agency says must be inked. Ink cards are $75.

Out-of-state fingerprint cards for travel nurses

When you apply for a nursing license in another state, that state's board usually can't receive an Oregon electronic fingerprint submission. Many boards ask out-of-state applicants to send FD-258 hard cards instead, which the board or its fingerprint vendor then processes.

California's Board of Registered Nursing requires FD-258 cards from applicants outside California and charges a separate hard card processing fee. Texas has out-of-state nursing applicants mail FD-258 cards to its fingerprint vendor for scanning. Nevada sends out-of-state applicants a fingerprint card to have completed locally. Illinois accepts two sets of prints on FD-258 cards from out-of-state applicants. Florida requires cards to be scanned and submitted electronically by an approved provider.

Requirements change and differ by license type, so always follow your board's current instructions. Some boards send their own cards, specify the ORI number and reason code to write on the card, or require prints to be taken by a law enforcement agency. Check those details before your appointment.

Tips for travel nurses

Start fingerprinting as soon as you begin a license application, because mailed cards can take weeks to process. Use a separate card for each state, with that board's ORI number and reason. Ask for an extra card in case one is rejected, and mail cards flat, without folding them.

Filling out the card

The top of the card holds your personal information, and some fields, such as the ORI number and reason for fingerprinting, come from the agency requesting your prints. Bring those details and we will help you complete the card neatly.

Insurance company filings

Insurance companies adding a board member, promoting an officer, or expanding into another state often need FD-258 cards for each person named in the filing, along with a notarized NAIC biographical affidavit. We handle both in one visit. See our insurance licensing page.

Free archive service

We keep your fingerprint record on file. If an agency rejects a card, asks for a second copy, or you need the same prints for another application later, we reprint or resubmit without a new appointment and without charging you again for the capture.

Agency, FBI, and card fees are set by those offices and still apply. Tell us the original appointment name and date when you call.

Questions about FD-258 fingerprint cards

How much are fingerprint cards?

Fingerprint cards are $55, and additional cards are $30 each. Ink cards, including FD-258, FD-1164, SF-87, and Ohio, Maryland, and New Jersey cards, are $75.

Do you do FD-1164, SF-87, or state fingerprint cards?

Yes. We roll ink prints on FD-1164 and SF-87 cards and on state cards for Ohio, Maryland, and New Jersey, as well as any other card that requires ink. Ink cards are $75.

Which states require FD-258 fingerprint cards from travel nurses?

Many state boards ask out-of-state applicants for FD-258 hard cards. Examples include California, Texas, Nevada, and Illinois, while Florida has cards scanned and submitted electronically by an approved provider. Requirements change, so check your board's current instructions.

Do I need a separate fingerprint card for each state license?

Usually, yes. Each board needs its own card with its ORI number and reason for fingerprinting, and boards generally can't share results with each other.

Can I use Oregon fingerprints for a California nursing license?

Yes, on FD-258 cards. California's Board of Registered Nursing requires hard cards from applicants outside California, plus its hard card processing fee.

Do I need to bring my own FD-258 cards?

If your board or agency sent you cards, bring those. If not, ask about blank FD-258 cards when you book.

How many FD-258 cards do I need?

It depends on the board or agency. Some ask for one card and others ask for two. Check your instructions, or tell us who the cards are for when you call.

What if I need another copy later?

We keep your fingerprint record on file, so reprints and resubmissions do not need a new appointment and we do not charge again for taking your prints. Agency and card fees still apply. Call (971) 514-3743 with the name and date of the original appointment.

People who book one of these often need another. All of them are available at our offices, and most by mobile appointment.
