Fingerprinting and affidavit notarization for insurance company filings

When an insurance company adds a board member, promotes an officer, or expands into another state, the filing usually needs two things from each person: a notarized NAIC biographical affidavit and, in many states, fingerprints on FD-258 cards. We come to your office, fingerprint everyone in one visit, and notarize the affidavits at the same table.

Independent private service

Oregon Apostille Fingerprinting Notary is a private fingerprinting and notary service. We are not the NAIC, a state insurance department, or an NAIC-approved independent third-party vendor, and we are not attorneys or compliance consultants.

Who the filing covers

Under the NAIC Uniform Certificate of Authority Application (UCAA), a biographical affidavit (Form 11) is required for all officers, directors, and key managerial personnel, and for anyone holding a 10 percent or greater ownership interest, including at the ultimate controlling person.

That list is usually longer than companies expect. Getting everyone in one room once is far easier than chasing signatures and appointments across a quarter.

The six-month clock

The affiant's signature and the notarization on an NAIC biographical affidavit generally cannot be more than six months old at the date the application is filed. Affidavits notarized too early have to be redone.

That's why we notarize and fingerprint in the same visit, timed to your filing date rather than to whenever the board happens to meet.

Fingerprint requirements differ by state

Some states require fingerprints in addition to the biographical affidavit, some accept the affidavit with an independent third-party verification report instead, and some have dropped fingerprints entirely. New York, for example, removed the fingerprint requirement for officers and directors and now relies on third-party verification.

The NAIC publishes a chart of fingerprint and biographical affidavit requirements by state, for both domestic and expansion filings. Send us the states you're filing in and we'll confirm what each one wants before we schedule anything.

A board visit, start to finish

Tell us how many people, where, and your filing deadline. We bring everything and set up in a conference room. Each person presents photo ID, we roll their FD-258 cards, check every impression before they leave the table, and notarize their affidavit while they're in front of us.

If some directors are out of state, we can fingerprint whoever is local and prepare cards for the rest to complete near them. Ask us and we'll explain what travels well and what doesn't.

Cards, not electronic submission

State insurance departments generally want physical fingerprint cards mailed with the filing rather than an electronic submission, which is why the FD-258 is still the standard here. We roll them in ink or print digitally captured prints onto the card, whichever reads more cleanly for that person.

Rejected cards are the most avoidable delay in this process. Faint prints from age or office work are common on boards, so we allow extra time rather than rushing a set that comes back.

What we don't do

We aren't an NAIC-approved independent third-party vendor, so we don't produce the background verification reports that accompany the affidavits. Those come from a vendor on the NAIC's approved list, sent directly to the department.

We also aren't attorneys or compliance consultants. We can't complete the UCAA, advise on your filing, or decide who needs an affidavit. We fingerprint, notarize, and hand you documents that are ready to file.

Free archive service

We keep your fingerprint record on file. If an agency rejects a card, asks for a second copy, or you need the same prints for another application later, we reprint or resubmit without a new appointment and without charging you again for the capture.

Agency, FBI, and card fees are set by those offices and still apply. Tell us the original appointment name and date when you call.

Questions about insurance licensing fingerprinting

Do you notarize NAIC biographical affidavits?

Yes. The affidavit is sworn and notarized, and the signature generally must be within six months of the filing date. We notarize at your office during the same visit as fingerprinting, at $10 per signature in Oregon and $15 in Washington.

Can you fingerprint our whole board in one visit?

Yes. Tell us the headcount, the location, and your deadline, and we'll quote the visit rather than charging per person travel. Call (971) 514-3743.

Which states require fingerprints for officers and directors?

It varies, and it changes. The NAIC publishes fingerprint and biographical affidavit requirement charts by state for domestic and expansion filings. Send us your filing states and we'll confirm before scheduling.

Do you submit the fingerprints electronically?

State insurance departments generally want physical FD-258 cards mailed with the filing, so that's what we prepare. If a state accepts electronic submission, tell us and we'll confirm what we can do.

Can you run the background check?

No. Background verification reports come from an NAIC-approved independent third-party vendor and go directly to the department. We handle fingerprinting and notarization.

What if a director lives out of state?

We can fingerprint everyone local in one visit and prepare cards for out-of-state directors to complete near them. Call and we'll work out the logistics.

How much does it cost?

Board and executive visits are quoted on headcount and location. Ink FD-258 cards are $75 each, and notarizing each affidavit is $10 per signature in Oregon.

What if I need another copy later?

We keep your fingerprint record on file, so reprints and resubmissions do not need a new appointment and we do not charge again for taking your prints. Agency and card fees still apply. Call (971) 514-3743 with the name and date of the original appointment.

People who book one of these often need another. All of them are available at our offices, and most by mobile appointment.
